Vital Records

PLEASE NOTE:  NEW MARRIAGE LICENSES REQUIRE AN IN PERSON APPOINTMENT.  CALL 696-5622

Certified copies of Birth, Marriage, and Death records are available at our office. The cost is $15.00 for the first copy and $6.00 for each additional one of the same certificate purchased at the same time. You may come into our office or order copies by mail.

Birth Records
How to request a certified birth record for a child born in Madison:

Write us a letter including the following

  1. Full name of Child, Including middle name
  2. Child's date of birth
  3. Child's Mother's First, Middle and Maiden names
  4. Child's Father's First, Middle and Last names
  5. Number of copies you would like 
  6. Signature and relationship to child
  7. A self-addressed stamped envelope
  8. Make check or money order payable to the TOWN OF MADISON
  9. Mail to: Town Clerk, P O Box 190, Madison, ME 04950
Death Records
How to request a certified copy of a death certificate:

Write us a letter including the following

  1. Full name of the Deceased, Including First, Middle and Last
  2. Date of Death
  3. Your relationship to the Deceased
  4. Number of copies you would like
  5. Signature of person requesting the certificate
  6. A self-addressed stamped envelope
  7. Make check or money order payable to the TOWN OF MADISON

The Office of Vital Statistics keeps birth, marriage and death records for each town in the state from 1923 - present  and also maintains divorce records from state courts. They will accept credit cards.
